Click on Evaluations | Surveys from the main menu.

To view evaluation forms or surveys that have assigned to you.

To view your evaluation forms. 

Use the title field to search for a specific survey.

Start searching.

Choose evaluations to view who will be evaluated.

You can export the table to Excel.

Choose to evaluate to start evaluating the form.

You can write a comment for each item.

Start evaluating based on items and indicators.

Submit your form.

You can write a general note here.
